CASE 1
Mr. M, a 69-year old Malay retiree, was admitted with complaints of pain in the upper abdomen for three days.The pain started approximately two hours after his meal, it was constant and dull aching. Change in posture and paracetamol did not relieve the pain. It was associated with fever, chills and rigor. He noted a change in his urine colour from yellow to dark tea colour.
Mr M. has diabetes mellitus for 6 years and has been very compliant to the treatment. He has not taken any NSAIDs and does not consume alcohol. On examination, he was febrile at 39o C, HR was 110bpm; BP was 120/80 mmHg; respiratory rate 23 breath per minute. The oxygen saturation was within normal range. The sclera was yellow and there was tenderness over the epigastrium.
